Table of contents missing in a docx4j generated document

Hi,
I'm attaching a document generated by docx4j. This document is not showing the table of contents when I expect it to show. Instead it gives me the message"Creating a table of contents? Start by applying heading styles from the styles gallery..." "No table of contents entries found." I expect te heading style to be set. Can you please help me validate what the issue here is?

I see the ToC instruction there: <w:instrText xml:space="preserve">TOC \h \z \u \t "Heading 1,1, etc...

So that much is ok. But as per the message, there is no content in your document for the ToC. Add some headings (ie paragraphs styled Heading1 etc):

Hi,
I'm attaching another document and this is the real issue. Only some of the Word users I am working with has the issue "No table of contents entries found." Rest including me are not getting the issue. ANy pointers would be great. Is it an issue with xml generated by docx4j or Microsoft who are consuming the metadata.
